CSC Speaker Application
Thank you for your interest in becoming a speaker at the Cancer Support Community of Greater Cincinnati and Northern Kentucky.
- All speakers must submit an application to our Speakers Bureau Committee for review and approval.
- All speakers must have two professional references. Please forward this CSC Speaker References page to your references to complete on your behalf.
To submit a request to speak at CSC, please fill out all of the form fields below, and upload your Head Shot Photo (.jpg) and CV/Resume at the bottom of the form.
To sign and certify your submission, please type in your name and date (this will be used as your official signature) and then click the “Submit” button to send us all of your information.
Your submission, along with the information provided by your two professional references, will be reviewed by our Speakers Bureau Committee.
